Scope and Contents

Mrs. Winship received a response from Carl Albert regarding her request for information about the application blanks for the Choc-Chic payments. He directed her to contact the Superintendent of the Five Civilized Tribes Agency in Oklahoma to obtain the necessary forms. Mrs. Winship then followed up with a question about whether she needed to fill out any forms or just provide her roll number.
