The Indian Student Enrollment Certification Individual Form, 1977-09-12
Scope and Contents
The "Indian Student Enrollment Certification: Individual Form, " (No. 506) is to be distributed to all students who claim Indian ancestry and who don't currently have one of these forms on file with the school. All information requested on the individual form should be supplied and signed by the parent or legal guardian, or by the student himself, if age eighteen.
Please note that it is not necessary to complete an additional enrollment form for Indian students who have a 506 form on file.
These two sentences give all indications to the Superintendent, or the person in charge of the operations of the Title IV-A program that you need a signed form on the Indian student who is claiming ancestry and that you never need to complete one again as long as that student has a contiguous attendance in that school district.
